Fairview EyeCare is committed to maintaining a safe and healthy workplace for our employees and our patients. As such, we are closely monitoring the ongoing COVID-19 caseload trend in our community and following the guidelines of the CDC (Centers for Disease Control and Prevention), as well as state and local government public health measures to flatten the curve. In compliance with these directives, there are a few New Rules which will guide your experience in our office that you should be aware of.
New Rules
Patients are requested to call or text the office upon arrival so entrance can be coordinated to minimize surface contacts and to not bring extra persons with them to an appointment, unless patient needs a caregiver or is a minor and needs one parent, in order to have no more than 10 persons inside, including staff on duty.
According to the CDC, this includes: people age 65 and older, people who are living in a nursing home or long-term facility, people who have weak immune systems, and people of all ages with underlying medical conditions. These patients are scheduled either 1st appointment of the morning or 1st appointment of the afternoon.
Patients are requested to wash their hands upon arrival to the office and before being escorted into the clinical care area.
Any patient who has contracted and recovered from COVID-19 must provide a copy of the negative lab test result, either from the lab or from their physician, prior to arrival at our office.
To try and limit the amount of time you are inside the office, patients are requested to complete all pre-appointment paperwork online via our secure patient portal. We’ll send your personal login/password when scheduling your appointment.
Within 2 hours of appointment time, patients will receive via text a 5 question safety survey regarding COVID exposure risk. This survey must be completed and received by the office for each person attending the visit, prior to being allowed to enter the building.
All doctors & staff members must wear masks and gloves when interacting with patients.
Patients are encouraged to wear their own masks/gloves when visiting our office.
Our office cannot provide PPE for patients due to limited replenishment availability.

Should a patient, visitor or staff member receive a diagnosis of COVID-19, the office will immediately report this information to the local health department for additional handling. All personnel will assist the local health department when possible in identifying potential contacts of a determined infected patient, visitor or staff member
